So..What the hell is a pilonidal cyst?

A pilonidal cyst (from Latin meaning 'hair nest') is a blanket term for any type of skin infection near the tailbone. These are normally quite painful, occur somewhat more often in men than in women, and normally happen in early adulthood. Although usually found near the tailbone, this painful condition can be found in several places, including the navel or the armpit. Development of the condition in a place other than the tailbone is exceedingly rare, however. It usually happens in young people, up to their thirties in age. Conditions in which it commonly occurs include obesity, body hair around the area in question, and a sedentary lifestyle. While a traumatic event is not believed to cause a pilonidal cyst, such an event has been known to inflame existing cysts."

It's so lovely let me tell you, and also warn you. I have apparently had this thing for YEARS. I was born with a "sacral dimple" which is exactly what it sounds like, a little dimple right at the top of my butt crack. No big deal right? A dimple is a dimple right? WRONG! My dimple is actually a pilonidal sinus cavity meaning it's basically an infected tunnel into my body.


NHS direct describes a pilonidal sinus as "a small hole or tunnel in the skin, usually at the very top of the cleft between the buttocks. Dents or pits in the skin can easily become filled with bits of skin and hair, and infected with bacteria. If the dent becomes blocked, it can get bigger, eventually forming a tunnel under the skin, filled with pus. This is known as a draining sinus or abscess."


So basically, what I thought was just a pimple that would come and go over the years was actually a cyst flaring up. This particular time it got so bad that it ruptured and became a pilonidal abscess. I finally got up the courage to see a doctor, who told me that if I had waited any longer I probably would have developed sepsis. The doctor was also shocked because I wasn't in any pain, whereas most people with this problem are in a lot of pain. They cleaned it up nice with peroxide, bactroban, and covered it with gauze. I was to be on augmentin for 10 days to clear the infection, with the dressing being changed twice every day.

I just saw my family doctor yesterday, who is referring me to a surgeon. I have to have this cyst and sinus cavity taken care of and healed before I can even think about having my lapband done. Yet another set back in my long list of setbacks. I emailed Sid Schwabb over at Surgeonsblog to see if he could blog about this for me, and he has some good knowledge of this subject as well. This is a part of his email to me:


"Once infected, a "cyst" is highly likely to reinfect; so whereas it's not mandatory to do anything about asymptomatic cysts/sinuses (meaning the initial dimple), after infection happens it's a good idea to take the plunge for curative surgery. Depending on what's done to accomplish it, there can be a pretty long healing process. A design flaw of the human body, I'd have to say. Big problems from a tiny hole!"

You have my sympathy, Pilonidal cysts/sinuses are no fun. I took it as nature's way of telling me that I sit on my hairy ass for too long.

My fiance has the little hole in the top op his butt/tailbone so i guess a sinus? What doctor should her go to a regular physician?

yeah just go to a regular physician, they tell you if it is actually a pilonidal sinus and then send you to the surgeon.
i agree they're no fun at all, i've had it 3 times and it seems to take longer to heal every time i have a surgery. but then again, this only happens to 10% of the people. i don't want to scare anyone. i'm just part of the unlucky 10%

My son was born with a little dimple above his butt crack, they first thought it was spinal bifida, but an U/S ruled that out. Thankyou for the information, I would have never even known about this if it wasn't for you sharing your story

I am 38 yrs old and my first experience with the Pilonidal cyst was when I was 21. I had the surgery to remove the cyst and then 14 yrs later the incision opened. I have since had 2 more surgeries to remove the sinus and once again I am in pain and the incision is open and bleeding. I am not sure what route to take next. I need some good advice PLEASE

Yeah I had to have surgery 3 times on my cyst and had to do laser hair removal just to get rid of the damn thing..its the most pain you will ever feel..sorry to scare you but just want to prepare you.

I've heard that for stubborn abscesses that recur again and again, there is a more complex surgery that actually creates a "flap" of sorts over your butt crack. There are a lot of variations on this surgery, called the Limberg Flap, Z-Plasty, Cleft Lift, stuff like that. These are all names of surgeries that have been tossed around throughout my research...I haven't looked too deeply into these because I don't have a recurring cyst.

i would recommend asking your doctor about one of these surgeries as an option. they are more complex than simple excision; they actually require a plastic surgeon to reconstruct your butt crack (for a lack of a better description).

about the cleft list I asked about it because I have reoccuring hell XD and basicaly you can get it done but you might have to go outside your insurance, I'm with kaiser and they basically told me I can't I feel all of your pain and good luck in the future! Oh and the cleft lift is they take a flap and basically move your insision and tissue to one side so it doesnt tear anymore, my skin would actually just tear apart... oh the joy

i have the same thing in picture. and i seriously dont no what to do im also embarass to tell anyone. i was just wondering if i dont get the surgery or anything is it like deadly does it cause any virus or cancer or stuff like that and is there any medication i can put on it

I have had 4 operation on my wound, and it still aint right as i write this now, i will also say it is the worst pain you will feel.

My son had the surgery in August, and he has to redo it. It opened a little and hair got in so it is irritated. We are going to redo it, but we are going to try the lazer hair removal. I have read that the hair needs to be darker than the skin. So hopefully his hair isn't to light. He has been ill for two years, finally feeling better, and now this thing. We will post the outcome.
